Job SummaryWe are seeking an experienced Night Shift Licensed Vocational Nurse (LVN) to join our team. The successful candidate will be responsible for providing direct patient care under the supervision of the Director or designee. This is a full-time position, working Monday through Friday, with a salary range of $80,000-$86,000 per annum.
Responsibilities- Triage phone calls from patients/families, ensuring prompt and effective communication.
- Maintain accurate and up-to-date records using electronic medical software systems (EMR), such as Allscripts.
- Work collaboratively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers to achieve positive patient outcomes.
- Develop and implement individualized plans of care, taking into account the unique needs and preferences of each patient.
- Participate in performance evaluations, identifying areas for improvement and implementing strategies to enhance job-related skills and knowledge.
- California LVN license required, and CPR certification must be maintained.
- A minimum of one year hospice experience is desired; two years SNF/home care experience is preferred.
- Knowledge of Hospice Medicare Conditions of Participation and California Hospice Licensing Standards is essential.
- Excellent communication and interpersonal skills are required, with the ability to work effectively with community physicians and other healthcare providers.
- A self-motivated individual who can work autonomously, set priorities, organize work, take initiative, and make independent decisions is necessary.
