Job Summary:
We are seeking a skilled Licensed Vocational Nurse (LVN) to join our healthcare team at Community Health Centers of the Central Coast. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the success of our medical team.
Key Responsibilities:- Provide direct patient care under the supervision of a licensed nurse or physician, including administering medications, performing vital signs, and documenting patient information.
- Assist with patient education and health promotion, including providing information on disease management, treatment options, and wellness strategies.
- Collaborate with the healthcare team to ensure seamless patient care, including communicating with providers, nurses, and other staff members.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Maintain accurate and up-to-date patient records, including medical history, treatment plans, and test results.
- Graduation from an accredited LVN program and current LVN licensure in the State of California.
- Minimum one year of recent experience in ambulatory care, preferably in a community health setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, with flexibility to adapt to changing priorities and patient needs.
- Proficiency in electronic health records (EHRs) and other healthcare software systems.
