About the Role

We are seeking a skilled Senior Healthcare Provider to join our team at Mountainside Residential Care Center in Margaretville, NY. As a key member of our multidisciplinary medical care team, you will be responsible for exercising autonomy in medical decision making and providing a broad range of diagnostic and therapeutic services.

Responsibilities
  • Conduct thorough physical examinations and assess patient histories to diagnose common illnesses or conditions among the elderly patient population.
  • Create and implement individualized care plans tailored to each patient's health needs and goals, in collaboration with the healthcare team and facility protocols.
  • Maintain accurate and timely electronic health records to document assessments, treatments, and care plans, ensuring compliance with regulatory standards and HIPAA guidelines.
  • Conduct regular rounds to monitor patient progress, address concerns, and provide on-call support during assigned hours for urgent situations.
Requirements
  • Board certification as a Physician Assistant or Nurse Practitioner.
  • New York State license (or eligibility).
  • A minimum of 1-year experience (new graduates with interest and abilities in surgery also encouraged to apply).
  • Excellent communication skills and ability to work collaboratively within a multidisciplinary team.

Estimated Salary Range:

$120,000 - $180,000 per year, based on qualifications and experience.
