About the Role

Careline Health Group is seeking a skilled Nurse Practitioner to join our team. As a key member of our multidisciplinary care team, you will provide primary and consultative professional care to geriatric and seriously ill patients in various care settings.

Key Responsibilities
  • Conduct thorough medical evaluations of elderly, frail, and seriously ill patients in their home environment.
  • Determine follow-up assessments based on patient need and best practice guidelines.
  • Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, physician, family, facility, and other personnel involved with patient care.
  • Assess health status by obtaining relevant health and medical history.
  • Perform physical exams based on age and patient history, as per the Standard Care Arrangement with a collaborating physician.
  • Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient clinical problems and healthcare needs.
  • Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges.
  • Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
  • Develop treatment plans in collaboration with patients and caregivers.
  • Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment.
  • Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas.
  • Provide patients and families with an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits.
  • Hold case conferences with families when appropriate to advance the care plan.
  • Meet minimum productivity requirements based on practice standards.
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
  • Master's or doctoral degree as a Board-Certified Nurse Practitioner.
  • Current State License as a Registered Nurse and Nurse Practitioner.
  • Nursing skills as defined by accepted nursing standards.
  • Sit, stand, bend, lift, and move intermittently.
  • Must be able to lift at least 35lbs and bear the weight of an average adult effectively.
  • Current and valid driver's license.
  • Reliable transportation and valid automobile insurance coverage.
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled-nursing facility settings.
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
  • Ability to handle multiple priorities, documentation requirements, and deadlines.
Preferred Qualifications
  • 2 years clinical experience in a post-acute or as a Nurse Practitioner.
  • Experience with geriatric patients.
  • Familiarity with Aprima EMR.
