We are seeking a compassionate and skilled Home Health Aide to provide high-quality care to our clients in their homes. As a Home Health Aide, you will work closely with our team to develop and implement personalized care plans that meet the unique needs of each client.

Key Responsibilities
  • Provide personal care assistance to clients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and laundry
  • Monitor and report any changes in client condition to the care team
  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements
  • Perform other duties as assigned by the care team
Requirements
  • Must be 18 years of age
  • Must be a Certified Home Health or Personal Care Aide with evidence of NYS Home Care Registry enrollment
  • Must have reliable transportation and a valid driver's license (if applicable)
  • Nurturing and compassionate nature with the desire to care for others
  • Ability to work with limited supervision and follow written and verbal instructions
