Job Title: Telephonic Triage LPN

Job Summary: We are seeking a skilled and experienced Telephonic Triage LPN to join our team. In this role, you will be responsible for providing high-quality nursing care to patients through telephonic communication.

Responsibilities:

  1. Collects and documents objective and subjective observations of the patient, telephonically and EMR, using approved clinical protocols to provide direction and disposition based on the patient's needs.
  2. Provides direction and support to patients and families, promoting health education and self-management skills.
  3. Collaborates with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.

Requirements:

  • Associate's Degree in Nursing
  • Licensed Practical Nurse
  • Acute Care 2-3 years Clinical Nursing
  • Strong computer skills
  • Ability to type and talk at the same time
