Behavioral Health Specialist

About the Role
CrescentCare is seeking a highly skilled Behavioral Health Specialist to join our team. As a key member of our care team, you will be responsible for completing behavioral health screenings and risk assessments, linking individuals to appropriate behavioral health services, and collecting behavioral health data required by grant funders.
Key Responsibilities
- Obtain clinical information for preliminary triage and risk assessment to determine acuity of needs for individuals requesting behavioral health services.
- Use standardized, validated, and culturally and linguistically-appropriate assessment tools to provide initial evaluation and identify immediate clinical care needs related to mental health and substance use disorders.
- Document screening and evaluation in the Electronic Medical Record and participate in multi-disciplinary team meetings to plan for the care of persons served.
- Assist with connecting patients with appropriate services to address their health-related needs and serve as the primary staff person providing direct services on the Substance Abuse and Mental Health Services Administration (SAMHSA) Promoting the Integration of Primary and Behavioral Health Care (PIPBHC) program.
Requirements
- Bachelor's degree in a mental health or related field or master's degree in a mental health field with practice regulated by a Louisiana Licensing Board.
- 1 year of work experience in a mental health facility or similar setting.
- RSW for bachelor's level social worker or CSW, LMSW, PLMFT, or PLPC for master's level social worker or counselor.
