Job Summary
As a Patient Care Navigator for Obstetric Medicine, you will play a critical role in ensuring the highest quality of care for patients attending the Collaborative Obstetric Medicine clinic and Postpartum Heart Care Program. Your primary responsibility will be to educate patients about the Postpartum Healthcare Program, including its benefits and services. You will also be responsible for calling patients if they have missed appointments, following up with patients after Emergency Department visits, checking in on patients who have mental health needs, reminding patients to do labs, reviewing lifestyle plans, performing screening for the social determinants of health, and connecting patients with resources such as transportation, diaper banks, community programs, etc.

We are seeking an experienced doula to work with our birthing families and provide prenatal and postnatal care. The ideal candidate will have a High School Diploma or GED, completion of approved DONA International doula training, and experience working with birthing families. A salary of $70,000 - $80,000 per year is offered, commensurate with experience.
