Job Title: Care Management Support Coordinator II Job Summary: BCForward is seeking a highly skilled Care Management Support Coordinator II to join our team. As a key member of our care management team, you will play a vital role in facilitating effective communication and coordination between members, providers, and staff. Key Responsibilities: * Conduct outreach to members via phone, assisting with care plan next steps, community resources, and scheduling inquiries. * Provide ongoing education to both members and providers to enhance understanding of care/services. * Connect members to health plan and community resources, ensuring access to high-quality customer service. * Serve as the first point of contact for member and provider inquiries, explaining care plan procedures and protocols. * Support member onboarding through administrative duties, including sending welcome letters, correspondence, and educational materials. * Document and maintain nonclinical member records, adhering to state and regulatory standards, and provide information to providers as necessary. * Utilize knowledge of local benefits and resources to address Social Determinants of Health (SDOH) needs through appropriate referrals. * Perform additional duties as assigned to support team objectives. * Comply with all organizational policies and standards. Requirements: * High School diploma or GED required. * 12 years of related experience preferred.
