Salary Information

The estimated salary for this role is $38.00-$60.00 per hour based on experience, with opportunities for annual increases and incentives.

Job Responsibilities
  • Plan, direct, provide, and document appropriate care for patients in the designated care setting.
  • Assist in coordinating comprehensive care for patients, including assisting with the consent process, data collection, and arranging follow-up visits, referrals, testing, treatment, and long-term follow-up.
  • Work collaboratively with the interdisciplinary team to coordinate ongoing care, focusing on cost-effective ways to maximize wellness and enhance patients' lives.
  • Develop, coordinate, and provide education for patients, family, caregivers, and staff based on personalized needs, serving as a resource for ongoing education.
  • Serve as a communication link between the patient, family, caregivers, interdisciplinary team, and outside hospitals/agencies.
Required Skills and Qualifications
  • Bachelor of Science in Nursing (BSN)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • CPR-Basic Life Support (BLS)
  • Minimum 2-3 years of recent and related clinical experience
