Job Description

Sacred Heart Home Health Care is seeking a dedicated and compassionate Home Health Nurse to join our team.

About the Role

This position involves providing high-quality nursing care to patients in their homes, working closely with them and their families to develop and implement personalized care plans.

  • Assess patient needs and create comprehensive care plans that meet those needs.
  • Provide skilled nursing services, including wound care, medication management, and education on self-care techniques.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements

To be successful in this role, you must have:

  • A valid nursing license in the state where practicing.
  • At least two years of experience in home health or public health nursing.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • CPR certification and a valid driver's license.
Compensation and Benefits

We offer a competitive salary range of $60,000 - $90,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
