Job Summary: This position works with Integral Care's ANEW Program at the clinic, providing continuity of care services to consumers who are re-integrating into the community after incarceration.

Key Responsibilities:

  • Provide services to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective.
  • Develop extensive knowledge of community resources and collaborate with interdisciplinary teams to cultivate a clinical skill set.
  • Forge therapeutic relationships with diverse community members and provide interventions and services that are both office and community based.
  • Work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes.
  • Participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals.

Requirements:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Current and valid driver's license and a clean driving record.
