Community Health Worker

The City of Providence is seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in conducting screenings and individual needs assessments in family homes and other community venues. Your primary responsibility will be to engage clients, implement screening tools, identify areas of need, develop action plans, and ensure referrals are made to appropriately matched services. You will also provide ongoing support and monitoring to clients.

Requirements:
- Community Health Worker Certification required; Bachelor's degree preferred.
- Ability to complete training and meet all requirements to be certified as a Community Health Worker within the first 6 months of employment.
- Spanish bilingual preferred.
- Valid driver's license, auto insurance, and reliable transportation.
- Ability to work independently and as an interdependent team member.
- Ability to provide services in clients' homes, health clinics, community locations, and other agency sites.
- Ability to have a flexible schedule to include some evenings.
