- Support wrap-around clinical services, including co-visits, telehealth visits, and independent LPN visits.
- Conduct health screenings, perform clinical services, and provide cross-coverage for team members as needed.
- Collaborate with interdisciplinary team members to improve care quality, reduce costs, and enhance patient and staff experiences.
- Current LPN licensure and certification in the state of employment.
- Experience providing care for individuals with mental illness and substance use disorders.
- Current CPR and BLS certification.
- Strong computer skills and ability to travel as needed.
- A competitive salary range of $34.00 - $38.00/hour with annual bonus.
