We are seeking an experienced Hospice Registered Nurse (RN) to join our full-time team in Chatham & Lee Counties.

Job Description:
  • Care Coordination: Apply your knowledge of nursing theories and concepts to provide comprehensive care and documentation that meets reimbursement guidelines.
  • Patient Advocacy: Coordinate physical care with patients, families, physicians, and the Hospice team while communicating effectively at all levels.
  • Collaboration: Work with agency personnel to ensure seamless patient care and participate in IDT meetings and quality reviews as scheduled.
Requirements:
  • Licensure and Experience: Current RN licensure in the state of practice, successful completion of a nursing education program from an approved school of nursing, and a minimum of two years of clinical nursing experience, preferably with at least one year of hospice nursing experience.
  • Knowledge and Skills: Familiarity with home health and hospice care programs, nursing practice acts, agencies' policies and procedures, and reimbursement guidelines.
  • Education and Certification: 12 hours of continuing education annually and CPR certification.
  • Flexibility: Willingness to accept travel assignments based on agency needs, including weekend, evening, and holiday call duties.
