We are seeking a skilled Nurse Practitioner (APRN) to join our team at Abode Care Partners, a leading provider of integrated medical services for seniors and individuals with complex needs.
About the RoleThis rewarding opportunity involves providing evidence-based medical care to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes. As a Nurse Practitioner, you will be responsible for examining and treating chronic and episodic illnesses, minor injuries, accidents, and other injuries or illnesses. Your expertise will also involve ordering and interpreting laboratory and diagnostic testing as needed, collaborating with caregivers and other healthcare professionals in providing interdisciplinary care, and maintaining accurate patient medical records.
Key Responsibilities- Develop and implement comprehensive care plans tailored to individual patient needs
- Conduct thorough physical examinations and provide evidence-based treatment recommendations
- Order and interpret laboratory and diagnostic tests as necessary
- Collaborate with multidisciplinary teams to ensure seamless patient care
- Maintain accurate and up-to-date patient medical records
To be successful in this role, you must have:
- An active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- A Master's degree in Nursing (MSN)
- Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills, both verbal and written
- Excellent time management skills with accountability for daily workflow
