Solvista Health seeks a skilled and compassionate Registered Nurse to join our team as a Patient Care Coordinator. This role is ideal for a dedicated healthcare professional who thrives in a fast-paced environment and is passionate about delivering exceptional patient care.
About the RoleThe Patient Care Coordinator will play a vital part in ensuring the smooth operation of our mental health clinic, working closely with patients, staff, and other healthcare professionals to provide comprehensive care. As a key member of our team, you will be responsible for coordinating patient care, developing and implementing individualized plans, and collaborating with multidisciplinary teams to achieve optimal patient outcomes.
Responsibilities- Coordinate patient care activities, including assessments, treatments, and follow-up appointments.
- Develop and implement individualized care plans, incorporating input from patients, family members, and healthcare professionals.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and address any concerns or issues that may arise.
- Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making and shared understanding.
- Provide education and support to patients and families on health-related topics, promoting empowerment and self-management skills.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- RN degree from an accredited nursing program, BSN preferred.
- Current state RN license and CPR certification required.
- A minimum of three years' experience in mental health nursing, preferably in an outpatient setting.
- Excellent communication, interpersonal, and organizational skills, with the ability to work independently and collaboratively within a team environment.
- Ability to maintain confidentiality and adhere to HIPAA guidelines at all times.
- Strong knowledge of nursing principles, practices, and laws, including state and federal regulations.
This is a full-time position, available seven days a week, 24 hours a day. You will be working in a fast-paced, dynamic environment, where adaptability and flexibility are essential. Regular travel to conferences, meetings, and branch locations may be required. The role involves moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.
Benefits- An estimated salary range of $70,000-$90,000 per year, depending on experience.
