Careline Health Group is seeking a skilled Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will provide high-quality patient care and support to terminally ill patients and their families in a home or facility setting.
Key Responsibilities:
- Complete initial evaluations and re-evaluate patient nursing needs during each visit.
- Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating patient plans of care.
- Provide support at the time of death and perform bereavement assessments if requested.
- Communicate effectively with patients, families, and healthcare teams.
Requirements:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse.
- Minimum one year experience as a nurse in a clinical care setting.
- Nursing skills as defined by accepted nursing standards.
