Job Overview

We are seeking highly skilled and compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our team as per diem Personal Care Attendant Program (PCA) Nurses.

In this role, you will play a vital part in supporting older adults, individuals with disabilities, and caregivers across Franklin, Hampshire, Hampden, and Worcester County's Montachusett Region.

Compensation and Benefits

Enjoy competitive hourly rates of $32/hour for LPNs and $34/hour for RNs, with an additional 6% compensation for bilingual skills.

Responsibilities
  1. Clinical Assessments: Conduct thorough assessments of individuals receiving MassHealth PCA Services, focusing on their functional limitations and medical conditions.
  2. MassHealth Application and Evaluation: Complete applications and evaluations in compliance with MassHealth regulations, ensuring timely submission.
  3. Collaboration and Teamwork: Work closely with Occupational Therapists, Functional Skills Trainers, and the PCA Program Director to ensure consistency and compliance in Evaluations.
  4. Communication and Documentation: Maintain accurate records, communicate effectively with consumers, caregivers, and supervisors, and adhere to HIPAA standards.
Requirements
  • Current Massachusetts RN/LPN License
  • At least one year of clinical experience in community or long-term care settings
  • Excellent assessment and evaluation skills
  • Computer proficiency for processing electronic records
  • Ability to work independently and collaboratively within an interdisciplinary team
Physical and Mental Demands

This role requires the ability to:

  • Communicate verbally with consumers, caregivers, and colleagues
  • Drive in various weather conditions to visit consumers' homes
  • Navigate uneven terrain and climb stairs
  • Lift up to 25 pounds occasionally
  • Maintain confidentiality and handle confidential information
Work Environment

You will split your time between indoor office work and traveling to meetings, trainings, and consumers' homes.

Please note that you must be able to tolerate various environmental conditions, including odors, pollutants, and seasonal temperature changes.
