About the Role

Pinnacle Home Care Inc. is seeking a skilled and compassionate Speech Therapist to join our team of healthcare professionals. As a Speech Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement individualized Speech Therapy treatment plans in consultation with physicians and revise as necessary.
  • Collaborate with other healthcare professionals to develop and implement interdisciplinary client care plans.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
  • Document all evaluation data, treatments, and patients' responses to therapeutic interventions in a timely manner.
  • Supervise Home Health Aides as needed and ensure they are equipped to provide high-quality care.
  • Report patients' progress and any changes in their condition to physicians and other healthcare professionals.
  • Evaluate the effectiveness of Speech Therapy interventions and update client care plans as needed.
Requirements
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Ability to work independently with minimal supervision and possess good observational, judgment, and communication skills.
