">Job Summary
">This role provides basic social services to individuals and/or families participating in the Healthy Start Program in Alachua County.
The responsibilities include case management for Healthy Start clients and education ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines.
Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.
">Key Responsibilities
">- ">
- Assist clients in accessing community services and provide information and education on smoking cessation, childbirth education, breastfeeding support, parenting, and infant growth and development.">
- Initiate tracking procedures for client, family of client, and client's provider to manage appointments and referrals for services.">
- Assess educational needs of Healthy Start clients at risk for poor pregnancy outcomes.">
- Complete all Healthy Start assessments and home visits as required per program guidelines.">
- Maintain/update client files and ensure Healthy Start database are updated as required per policy and Healthy Start Standards and Guidelines.">
- Conduct outreach activities including home visits to clients to provide Healthy Start services and to providers and other community organizations to promote the program.">
- Participate in quality improvement and quality assurance activities of the program to enhance services.">
- Other duties as assigned.">
Requirements
">To be successful in this role, you will need:
">- ">
- Bachelor's degree in Social Sciences, Social Work, Human Services, or a Health Related Field such as Health Education, Child Maternal Health, or Public Health.">
- 2 years of related experience preferred.">
- Experience conducting home visits with similar populations preferred.">
- Bilingual (Spanish) preferred.">
- Valid Florida drivers' license and insurable by agency's insurance carrier.">
- Must successfully clear Level II Background Screen.">
- Must possess an acceptable driving record and have reliable transportation.">

- An estimated salary of $45,000-$60,000 per year based on location and experience.">
