Job Opportunity at Mountain Ridge Center

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Mountain Ridge Center in Franklin, NH. As an LPN MDS Coordinator, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities:
  • Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process.
  • Participate in developing individualized resident Care Plans.
  • Train new members of the interdisciplinary team on clinical content and electronic completion of the applicable MDS items.
Requirements:
  • Must be an LPN in good standing and currently licensed by the state.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures.
