Job Summary

We are seeking a skilled Medical Social Worker to join our team in the ambulatory care setting. The ideal candidate will have a Master of Social Work degree and at least one year of recent medical or mental health experience.

Key Responsibilities
  • Deliver care management and behavioral health services to patients across the lifespan
  • Coordinate care to ensure patient/family access to community services
  • Administer behavioral health screenings and conduct assessments as needed
  • Develop individualized patient-centered care plans and monitor progress
  • Deliver brief interventions to support lifestyle and behavioral changes
  • Respond to crisis situations in the ambulatory setting
Requirements
  • Master of Social Work degree or equivalent
  • At least one year of recent medical or mental health experience
  • Licensed Master Social Worker (LMSW) in the state of Michigan preferred
  • Limited Licensed Master Social Worker (LLMSW) in the state of Michigan will be considered
