Home Health RN Case Manager

At Heartland Hospice, we are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
About the Role
As a Home Health RN Case Manager, you will be responsible for administering skilled nursing care to clients requiring professional services in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your primary focus will be on providing patient-centered care, promoting independence, and enhancing the overall quality of life for our patients and their families.
Responsibilities
• Provide direct patient care services to patients and supportive care to the patient's family with a reasonable case load
• Act as primary liaison between physician, patient/family and home health team
• Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
• Maintain all assigned patient clinical records
• Participate in company quality assessment and performance improvement programs
Requirements
• Compassionate, kind and empathetic to your patients and their families
• RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
• Current driver's license and automobile insurance
• Current CPR certification
