About the Role

We are seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team at Hospice Of Cleveland County Inc. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with advanced life-threatening or life-limiting illnesses.

Key Responsibilities
  • Provide comprehensive medical care to patients, including pain and symptom management, and coordination of care with other healthcare professionals.
  • Conduct patient and family assessments, develop and implement individualized care plans, and provide education and support to patients and their families.
  • Collaborate with the medical director and other healthcare professionals to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
  • Stay up-to-date with current trends and best practices in palliative care and hospice medicine, and apply this knowledge to improve patient care and outcomes.
Requirements
  • Master's degree in nursing with a focus on palliative care or a related field.
  • Minimum 1 year of experience in direct care of patients with life-limiting illnesses, preferably in a hospice or palliative care setting.
  • Current licensure as a registered nurse (RN) and certification as a nurse practitioner (NP) in a relevant specialty.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
