Trinity Health Corporation is seeking a dedicated Registered Nurse to join our dynamic Nursing Team at Saint Mary Home.

What to Expect:

A Clinical Care Coordinator at Trinity Health plays a vital role in delivering safe, quality patient care. As an active participant in the care team, you will ensure individual resident needs are met and choices honored. You will lead the interdisciplinary care team to achieve goals and collaborate with colleagues to provide exceptional care.

Responsibilities:

This position involves supervising nursing assistants, directing care, and participating in care planning. To excel in this role, you should possess excellent observation and teamwork skills, along with recent clinical experience in long-term care or acute care settings. A current CPR certification is also required.

Requirements:

To be considered for this position, you must have a graduate degree from an accredited school of nursing and hold a current licensure as a Registered Nurse in the State of CT. Recent clinical experience and a strong commitment to providing exceptional patient care are essential for success in this role.
