Community Case Manager

St. Anthony's is seeking a skilled Community Case Manager to join our Resource Center team. As a key member of our social service team, you will provide critical support to individuals experiencing homelessness and extreme poverty.
Key Responsibilities- Conduct initial screenings, intakes, and needs assessments to develop effective service plans.
- Maintain accurate and up-to-date electronic files to document case work progress.
- Assist clients in accessing benefits and other eligible services.
- Provide Payee services to ongoing guests and complete Rental Assistance applications as assigned.
- Work collaboratively with other St. Anthony's teams and outside agencies to deliver comprehensive services.
- Attend case conferences and participate in required trainings and meetings.
- Enter assessments, guest data, and progress notes into our centralized data collection system.
- Three or more years of experience in social service work, preferably with low-income or homeless populations.
- Case management background and a BA or equivalent experience are desired.
- Ability to work effectively as a team member and set appropriate limits and behavioral boundaries.
- Computer literacy in Windows and MS Office, as well as strong oral and written communication skills.
