About the Role
We are seeking an experienced Physical Therapist to join our team at Civita Care Center at Long Ridge - Stern Rehab in Stamford, Connecticut. This is a travel contract opportunity with a 13-week duration.
Job Summary
The successful candidate will be responsible for managing treatment plans to promote positive outcomes for patients with diverse diagnoses in a skilled nursing facility setting.
Duties and Responsibilities
- Develop and implement individualized treatment plans to meet patient needs and goals
- Provide skilled physical therapy services to patients, including assessments, interventions, and evaluations
- Collaborate with multidisciplinary teams to ensure comprehensive patient care
- Maintain accurate records and documentation of patient progress
Requirements
To be considered for this role, applicants must have:
- A current BLS (AHA Preferred)
- An active State License
- Supervisory professional references
- One year of recent experience in a skilled nursing facility
