Hospice Nurse Case Manager

As a full-time Hospice RN Case Manager, you will have the opportunity to provide direct patient care, manage an interdisciplinary team, and ensure that each patient receives the personalized care they deserve.
Key Responsibilities:
Provide direct patient care and manage an interdisciplinary team

Collaborate with healthcare professionals to develop and implement patient care plans
Requirements:
Compassionate and empathetic to patients and families
Active RN license in the state of employment
1+ years of nursing experience
Current CPR certification
