Ardor Health Solutions is seeking a skilled Occupational Therapist to fill a travel position in a Home Health setting in Eureka, California. This is a full-time contract position that offers a unique opportunity to work with a dynamic team and provide exceptional care to patients in their homes.

Key Responsibilities:

  • Provide occupational therapy services to patients in their homes, focusing on helping them develop, recover, or maintain the skills needed for daily living and work.
  • Conduct thorough assessments of patients' needs and develop personalized treatment plans to address their goals and objectives.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress and outcomes, maintaining accurate and detailed records of treatment plans, progress notes, and discharge summaries.

Requirements:

  • Active California Occupational Therapist license
  • 2+ years of experience as an Occupational Therapist, preferably in a Home Health setting
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care and outcomes
