About the Role

We are seeking a highly skilled Medical Care Navigator to join our team at Springfield Medical Care Systems, Inc. This is an exciting opportunity to make a meaningful impact in the lives of our patients.

Job Summary:

The Medical Care Navigator will work closely with our healthcare team to provide exceptional patient care and support. Key responsibilities include:

  • Assessing patient needs and developing personalized care plans to achieve optimal health outcomes;
  • Collaborating with patients, families, and healthcare providers to ensure seamless transitions of care;
  • Facilitating access to medical services, specialty care, and community resources to meet patient needs;
  • Providing education and support to patients and their families on disease management, self-care, and healthy behaviors;
  • Participating in quality improvement initiatives to enhance patient care and satisfaction.

Requirements:

To be successful in this role, you will need:

  • At least 2 years of experience in nursing or a related field;
  • Excellent communication and interpersonal skills;
  • Proficiency in Microsoft Office and electronic medical records systems;
  • A current LPN license and 1-2 years of experience in case management (preferred).

What We Offer:

We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision insurance, retirement plan, and paid time off.
