Job Summary

We are seeking an experienced and compassionate Clinical Care Coordinator to join our team in New Hampshire. As a key member of our care management team, you will be responsible for assessing, planning, implementing, and evaluating patient care.

Responsibilities

  1. Patient Assessment
  • Evaluate patients for clinical, psychological, and social factors that may contribute to their disease process.
  • Determine the appropriateness of patient status, severity of illness, and intensity of treatment.
Care Planning and Coordination
  • Develop and implement comprehensive care plans to address patient needs.
  • Collaborate with healthcare teams to ensure seamless transitions of care.
Utilization Review
  • Conduct regular reviews to determine medical necessity and appropriateness of patient care.
  • Identify areas for improvement and develop strategies to enhance patient outcomes.
Communication and Documentation
  • Provide clear and concise communication with patients, families, and healthcare teams.
  • Maintain accurate and detailed documentation of patient care and progress.

Requirements

To be successful in this role, you will need:

  • A minimum of 2 years of experience in acute care or care management.
  • A current NH RN license and certification in a relevant specialty (e.g., CCM, ACM).
  • Excellent interpersonal and organizational skills.
  • Ability to work effectively in a fast-paced environment.

Salary Range

The estimated salary for this position is $65,000-$85,000 per year, depending on experience and qualifications.
