Licensed Practical Nurse

Penn Highlands Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Coordinator. In this role, you will work closely with physicians and mid-level providers to develop, implement, and evaluate patient care plans in our office setting.
Key Responsibilities:
Coordinate patient care with other healthcare services and providers
Assist in developing and implementing patient care plans
Serve as a liaison between healthcare providers and patients
Requirements:
High school diploma or equivalent required
Current LPN licensure in Pennsylvania required
Knowledge of regulatory requirements for third-party payors preferred
CPR certification from the American Heart Association within the first year of hire required
