Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to serve as a Charge Nurse in our long-term care, dementia, and sub-acute units. As a Charge Nurse, you will be responsible for providing direct nursing care and services to our residents, while also supervising and directing the work activities of assigned clinical staff.
Key Responsibilities- Provide treatment, care, and rehabilitation services to residents in accordance with individualized care plans.
- Administer medications and manage pain effectively, ensuring timely and accurate administration.
- Implement, evaluate, and provide nursing services as directed by management.
- Monitor skin conditions and provide wound care as necessary.
- Accurately observe, assess, and document resident signs and symptoms, changes in condition, behaviors, reactions, and findings.
- Ensure compliance with established infection control procedures and state/federal regulations.
- Assign and direct the work activities of assigned clinical staff.
- Provide feedback to supervisors regarding staff performance.
- Assist with staff coverage of shifts, making assignments appropriate to staff skill levels.
- Evaluate staff performance, recommending retraining, coaching, or corrective action as needed.
- Provide acceptable standards of resident care and services based on established resident rights and person-centered care.
- Identify problems and report to supervisors and/or administration as necessary.
- Maintain a working climate that promotes growth and job satisfaction among staff.
- Accurately and promptly implement physician orders.
- Critically think, be resourceful, and remain calm in unique and emergent situations.
- Provide a complete report to on-coming nurses of resident information, needs, and changes in condition.
- Maintain professional behavior and appearance with confidentiality.
- Provide excellent customer service, as evidenced by concern and initiative to problem-solve and exceed resident and customer expectations.
- Observe and support RestHaven policies and procedures.
- Float between units as needed.
- Assume other position responsibilities as assigned.
- Communicate effectively and tactfully, recognizing age, cultural orientation, needs, abilities, and physical condition, while using appropriate interventions.
- Establish and maintain effective and professional working relationships with residents, families, visitors, and employees.
- Be well-organized and attentive to detail.
- Practice the highest standards of ethics, honesty, and accuracy.
- Work independently, as an effective team member, and manage time productively.
- Maintain confidentiality in all aspects of the Care Center, its residents, and employees.
- Have proficient interpersonal relations.
- Have effective written and verbal communication skills.
- Be proficient in computer skills and abilities.
- Demonstrate clinical competence.
- High school graduate or successful completion of GED.
- Graduation from an accredited Licensed Practical Nurse or Registered Nurse program.
- Successful completion of the National Association LPN or RN Michigan State boards.
- Previous nursing experience, especially with the elderly population, is preferred.
- Meet ongoing requirements to maintain current nursing license.
- Assume responsibility for own continued personal and professional growth.
- Participate in learning and development opportunities.
- Attend all required in-service training and departmental meetings.
- Possession of a current license from the Michigan Board of Nursing to practice as a Licensed Practical Nurse or Registered Nurse.
