Salary: $90-$95 per completed Assessment

We are seeking a skilled Nurse Practitioner- PRN/Part Time to join our team in Marion, Ohio. As a member of our team, you will conduct comprehensive health assessments in various settings, including home visits, skilled facilities, and telehealth appointments.

About the Position:

You will review and observe patients' current health, medical history, medication adherence, social environment, and other risks during each visit. This provides valuable insight into their overall health and well-being, which can be difficult to capture during routine office visits. Your work will contribute significantly to improving quality of care and closing potential care gaps.

Responsibilities:

As a Nurse Practitioner with Matrix Medical Network, your key responsibilities include:

  • Conducting adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
  • Collaborating with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel as needed.
  • Working with primary care physicians on patient education and follow-up.
  • Providing services in various settings, including home visits, skilled facilities, and telehealth appointments.

Qualifications:

To be successful in this role, you must meet the following requirements:

  • Master's Degree or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Ability to travel and possess a valid state driver's license.
