Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at St. Mary's Home Care & Hospice. As a Contingent Home Care RN, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional patient outcomes.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes
Requirements
  • Graduate of an accredited nursing program
  • One year of experience as a professional acute care nurse
  • Home care experience preferred but not required
  • Ability to work independently and as part of a team
