Job Summary

We are seeking a skilled Senior Geriatric Nurse Practitioner to join our team in Manalapan, NJ.

About the Role

This is a challenging and rewarding opportunity for an experienced nurse practitioner to provide high-quality patient-centered care in nursing homes and assisted living facilities.

Responsibilities:
  • Clinical Assessment and Care Planning: Conduct thorough clinical assessments of residents and develop individualized care plans to improve health outcomes.
  • Primary and Urgent Care Services: Provide on-site primary and urgent care services to meet the needs of residents.
  • Patient Monitoring and Adjustment: Monitor high-risk patients and adjust care plans as needed to ensure optimal care.
  • Collaboration and Communication: Work collaboratively with primary care physicians, facility staff, and resident families to ensure seamless communication and care coordination.
Requirements:
  • Licensure and Experience: Active Nurse Practitioner license in New Jersey and a minimum of 1+ years of experience in geriatrics or long-term care.
  • Skillset: Strong communication, teamwork, and problem-solving skills.
