Job Overview:

We are looking for a dedicated and experienced Registered Nurse to provide behavioral health visits to patients in the Stratford, Milford, and Bridgeport areas. This is a unique opportunity to work with a supportive and collaborative team, making a positive impact on the lives of our patients.

About the Role:

This role involves conducting home visits to provide nursing care and support to patients with behavioral health needs. You will work closely with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:

  • Providing high-quality nursing care and support to patients in their own homes
  • Conducting assessments and developing care plans to meet the unique needs of each patient
  • Working collaboratively with other healthcare professionals to ensure seamless care delivery
  • Maintaining accurate and detailed records of patient interactions and care provided
  • Participating in ongoing education and training to enhance clinical knowledge and skills

Requirements:

  • Degree in Nursing (RN) from an accredited institution
  • Current RN license in good standing
  • CPR certification with American Heart Association or American Red Cross
  • Ability to meet physical demands of the job
  • Positive attitude and strong communication skills
  • Dedication to quality patient care
