Job Summary:

Camcare Health is seeking a highly skilled and compassionate Family Nurse Practitioner to join our team in providing primary care services to the patients of CAMCARE HEALTH CORPORATION. As a key member of our healthcare team, you will be responsible for coordinating and managing individual patient care, working collaboratively with staff physicians, and providing continuing care to assigned patients in the office setting.

Key Responsibilities:
  • Coordinates and manages individual patient care in collaboration with the Clinical Director of and staff physicians.
  • Works cooperatively with all physicians and staff to carry out the goals and objectives of CAMCARE HEALTH CORPORATION, according to established policies and procedures.
  • Provides continuing care to assigned patients in the office setting, anticipating, and recognizing deviations from normal, initiating treatment, and consulting with a physician as appropriate.
  • Orders diagnostic tests, medications and lab tests as indicated. Analyzes, interprets, and synthesizes data and applies information to patient management.
  • Provides education and counseling for patients, families, and community groups on various health conditions.
  • Functions as a member of a multi-disciplinary health care team.
  • Assists with the development, implementation, and monitoring of clinic policies, procedures, and protocols, as needed to achieve CAMCARE HEALTH CORPORATION goals and objectives.
  • Assists with the development, implementation, and monitoring of CAMCARE HEALTH CORPORATION Performance Improvement program.
  • Attends and actively participates in departmental and medical staff meetings as required.
  • Fulfills position responsibility in a manner commensurate within the mission and objectives of CAMCARE HEALTH CORPORATION.
  • Meets all attendance and punctuality requirements to ensure proper coverage and quality service.
  • Always demonstrates professional demeanor and behaviors, when interacting with staff, patients, and community.
  • Supports policies, procedures, and supervisory decisions.
  • Utilizes established channels of communication and lines of authority for problem solving and communication.
  • Demonstrates an ability to resolve interpersonal and professional conflicts appropriately.
  • Ability to develop and maintain cooperative and effective working relationships with the CEO, CMO, and other members of the Management Team, Board, and community groups.
  • Keeps all matters related to the organization confidential in compliance with confidentiality policy.
  • Provides proper notification of absence or tardiness.
  • Appearance and dress are neat and orderly, consistent with professional status.
  • Performs other duties and accepts other responsibilities as assigned.
Requirements:
  • Patient-focused compassionate team player.
  • A passion for helping patients.
  • Active State of New Jersey RN and Advanced Practice Nurse License.
  • Active State of New Jersey CDS and DEA License to prescribe and maintain controlled substances.
  • Certification by the New Jersey State Board of Nursing as an Advanced Practice Nurse.
  • Active BLS certification.
  • Master' Degree in Nursing from an accredited School of Nursing.
  • Maintaining continuing educational requirements as required for licensure and skills maintenance.
  • 2-4 Years' Experience at APN level in outpatient medical center or long-term care setting (preferred).
