In this role, you'll have the opportunity to:

Manage Chronic Disease and Diagnose Illnesses:
Deliver high-quality patient care in post-acute settings, working collaboratively with interdisciplinary teams to manage chronic disease, diagnose illnesses, and develop personalized plans of care.

Collaborate with Clinicians and Leaders:
Build relationships with facility leaders, clinicians, and other healthcare professionals to ensure seamless care coordination and delivery.

Mentorship and Leadership Opportunities:
Provide guidance, oversight, and mentorship to our nurse practitioners, helping them grow in their careers and reach their full potential.
