Nurse Consultant for Home Health Services

We are seeking an experienced Certified Home Health Registered Nurse to join our team as a Nurse Consultant for Home Health Services. As a key member of our care team, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Your primary duties will include:

• Conducting comprehensive patient assessments and developing individualized plans of care in collaboration with our multidisciplinary team
• Providing patient episode management according to protocol and ensuring continuity of care through timely and effective communication with all team members
• Educating patients, caregivers, and family members on aftercare or ongoing care requirements
• Performing vital signs and patient assessments during visits

To be successful in this role, you will need:

• A current and unencumbered RN license
• 1+ year(s) of experience as a Certified Home Health Registered Nurse
• OASIS experience and strong assessment, critical thinking, and communication skills
• Current CPR certification and comfortability using technical tools and software/EMR for visit documentation
• Reliable transportation, current driver's license, and car insurance coverage
• Proof of immunization/exemption for flu and COVID-19 and TB test or chest X-ray
