We are seeking a skilled Registered Nurse to join our team in Rockford, IL. As a Home Health Infusion RN, you will have the flexibility to create your own schedule and travel within the area to meet patients' needs.

Job Summary

This is a full-time, permanent position that requires 3 years of RN experience and 24/7 accountability. You will work with patients who have implanted intrathecal drug delivery devices, providing comprehensive patient assessments, nursing interventions, pump refills, dose adjustments, and patient education.

Responsibilities
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Provide nursing interventions, pump refills, and dose adjustments as needed
  • Collaborate with patients, caregivers, and healthcare teams to ensure seamless care
  • Document patient information accurately and maintain confidentiality
  • Participate in shared governance and integrate evidence-based research into practice
Requirements
  • Current licensure as a Registered Nurse
  • 3 years of RN experience with 24/7 accountability
  • Ability to work independently and as part of a team
  • Strong written and verbal communication skills
  • CPR certification
Preferred Qualifications
  • ICU/ER/OR and/or home care experience
  • Solid understanding of chronic pain/rehab principles
  • BSN
