**About the Role:**

We are seeking a skilled and compassionate Registered Nurse to join our team at Backus Hospital. As a Clinical Care Coordinator, you will play a vital role in delivering high-quality patient care and ensuring seamless coordination of services.

 

**Job Summary:**

Demonstrate clinical competence through utilization of the nursing process and development of a synergistic nurse-patient relationship. Assess and identify patient problems/needs, establish a plan of care, implement and evaluate care provided to patients.

 

**Responsibilities:**

Assessing Patient Needs:

  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized plans of care to meet those needs.

Coordinating Care:

  • Collaborate with interdisciplinary teams to ensure comprehensive care planning.
  • Cross-train on relevant medical specialties to enhance patient outcomes.

Education and Qualifications:

  • Graduate of a school of nursing.
  • One (1) year acute RN experience.
  • Current unrestricted license in good standing to practice as a Registered Nurse in the State of Connecticut.
  • Current Basic Life Support (BLS).
  • Ability to obtain and maintain department specific certifications.

Salary Range: $85,000 - $110,000 per annum, depending on qualifications and experience.
