The ideal candidate will serve as a liaison between individuals, communities, and health and social services to facilitate access to care, improve service delivery, and address social determinants of health.

Key Responsibilities:
  • Provide telephonic and in-person support to patients with barriers to accessing healthcare.
  • Connect patients with community resources and services.
  • Facilitate communication between patients, healthcare providers, and social services.
  • Assess and address social determinants of health that impact patient outcomes.
Requirements:
  • High school degree or equivalent.
  • 1+ years of experience in a healthcare setting.
  • Proficient computer skills to work with electronic medical records.
  • Ability to obtain Community Health Worker Certification within the first year of employment.
