Job Summary

We are seeking a highly skilled and compassionate Per Diem Nurse Practitioner/Physician Assistant to join our team at Optum Home and Community Care. As a key member of our Senior Community Care (SCC) product, you will provide high-quality care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities
  • Deliver cost-effective, quality care to assigned members, managing both medical and behavioral, chronic and acute conditions effectively.
  • Perform comprehensive assessments and document findings in a concise and compliant manner, ensuring accurate diagnoses and coding.
  • Facilitate agreement and implementation of the member's plan of care, engaging facility staff, families, primary and specialty care physicians.
  • Coordinate care as members transition through different levels of care and care settings, monitoring needs and facilitating adjustments to the plan of care.
  • Review orders and interventions for appropriateness and response to treatment, identifying the most effective plan of care that aligns with the member's needs and wishes.
Requirements
  • Certified Nurse Practitioner or Physician Assistant through a national board.
  • Active and unrestricted license in the state of practice.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Access to reliable transportation and ability to travel to client and/or patient sites.
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.
Preferred Qualifications
  • 1+ years of hands-on post-grad experience within Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
