Job Summary

We are seeking a highly skilled Patient Care Navigator to join our Emergency Department team at Novant Health.

  • Department: Emergency Department
  • Schedule: 4x10's, 7:00 am to 7:00 pm
  • Location: Thomasville Medical Center, Thomasville, NC

The Patient Care Navigator plays a critical role in ensuring seamless patient care throughout their emergency department experience. This dedicated professional serves as an ongoing resource, providing comfort, education, and direction to patients and their families during this critical period.

Key Responsibilities:
  • Communicate effectively with healthcare teams, patients, and their families to ensure comprehensive care
  • Collaborate closely with care team members to identify and sequence care that meets patient needs
  • Respond to healthcare-related inquiries, facilitating patient requests and providing clinical assistance as necessary
  • Maintain open, honest, and timely communication among all parties, fostering a supportive environment for patients and their families
