We are currently seeking skilled Home Health Nurses to join our team in providing continuous care to patients in their homes. This role offers a unique opportunity to make a positive impact on the lives of our patients and their families.

**Job Summary:**

  • Implement physicians' prescribed plans of care for patients in their homes
  • Conduct thorough assessments and coordinate care with healthcare teams
  • Administer medications, treatments, and therapies as prescribed
  • Document all care provided in our system of record

**What You'll Need:**

  • An active, unencumbered nursing license (RN/LPN/LVN) in the state where you'll practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test within the last 12 months
  • At least six months of prior hands-on nursing experience
