Housing Stability Coordinator

Housing Up is seeking a dedicated and compassionate Rapid Re-Housing Case Manager to join our team. As a key member of our housing team, you will play a vital role in helping families achieve stability and independence.

Job Description:
As a Rapid Re-Housing Case Manager, you will be responsible for providing home-based case management services to families who have recently experienced homelessness. Your primary goal will be to assist families in addressing barriers to maintaining permanent housing using a strengths-based, Housing First approach. Key functions include:
• Coordinating with our housing team to locate housing options for families
• Developing and updating family goal plans with program participants, assisting them in setting and prioritizing short- and long-range goals
• Conducting monthly home visits to support families in maintaining housing stability, including on-time rent payments and household maintenance
• Working with families to resolve issues with landlords, as needed
• Providing referrals to Housing Up's supportive services (clinical and housing services) and collaborating with the team to advance client goals
• Maintaining accurate records in the Homeless Management Information System (HMIS) database and other case files
Required Skills and Qualifications:
To succeed in this role, you will need:
• A Bachelor's degree in human services or a related field (preferred) or a high school diploma/equivalent and 3 years of direct service experience
• Valid driver's license/reliable transportation required
• Two years of experience in the social/human services field (preferred)
• Experience working with families, homelessness, mental health, and/or substance abuse that reflects an understanding of the causes of homelessness
• Working knowledge of the strengths-based, client-centered approach
• Demonstrated cultural competence
• Excellent interpersonal and conflict resolution skills
• Strong oral and written communication skills
• Knowledge of community resources available to families

Estimated Salary Range:
$52,500 - $57,000 per year, depending on qualifications and experience.
