Physical Therapist

We are seeking a skilled Physical Therapist to join our team at PACS Inc. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Develop and implement effective treatment plans for patients, working closely with referring physicians and other healthcare professionals.
- Treat patients according to established treatment plans, utilizing your expertise in physical therapy to promote optimal patient outcomes.
- Collaborate with the nursing department to provide training and support to Restorative Aides, ensuring seamless care coordination.
- Supervise Physical Therapy Assistants in direct patient care and related activities, adhering to state practice acts and regulatory requirements.
- Communicate effectively with supervisors and other healthcare team members regarding patient progress, challenges, and plans.
- Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed, contributing to the development of comprehensive care plans.
- Engage in in-services training programs for other staff members, promoting knowledge sharing and best practices.
- Record and maintain accurate patient evaluations, treatment notes, progress reports, and recertification documentation, ensuring compliance with PT Board state practice acts and governmental regulations.
- Provide education and training to patients' families or nursing staff on maintenance programs and caregiver training, preparing patients for discharge from therapy services.
- Participate in discharge planning, ensuring a smooth transition for patients and their families.
- Recommend and facilitate the ordering of necessary durable medical equipment to promote patient independence and mobility.
- Report any equipment issues to ensure timely maintenance and optimal functionality.
- Exemplify excellent communication and interpersonal skills, fostering positive relationships with patients, families, staff, and professional colleagues.
- Valid Physical Therapist license
