Hospice Care Specialist

As a Hospice RN/RN Case Manager, you will provide direct patient care services, act as a liaison between physicians, patients, and families, and manage an interdisciplinary team to provide comfort, peace, and dignity. The ideal candidate has a hospice heart, is comfortable providing care in various settings, and possesses 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
